You have selected the PREMIUM CONTACT LENS PLAN (yearly) membership level.

Please Note: 

  • Patient is immediately eligible to receive all four examinations

  • Patient is immediately eligible to order glasses after exam

  • Once patient receives glasses, no refund is available for yearly membership fee

  • patient will receive contact lens supply in 90-day increments after quarterly exams

The price for membership is $1,500.00 per Year.

Membership expires after 12 Months.
